Payment Policy
At Lakeville Family Dental Center, we are committed to providing high-quality care with clear and transparent financial policies. We appreciate your understanding and cooperation as we work together to make your experience as smooth as possible.
Credit Card Processing Fee
Due to increased processing costs, a 2.8% surcharge will be applied to all credit card transactions beginning July 1, 2025. This fee helps offset the rising cost of credit card usage.
There is no surcharge for the following payment methods:
Cash
Check

For Patients with Dental Insurance
For patients with insurance, a portion of treatment costs is due at the time of service.
Payment Guidelines:
20% due for basic restorative treatment
50% due for major restorative treatment
Because insurance plans and benefits vary widely, we may not always know your exact coverage in advance.
For non-preventive or non-urgent treatment, we are happy to submit a pre-treatment estimate to your insurance provider. This allows you to better understand your expected benefits and out-of-pocket costs before beginning treatment.
Preventive Care & Diagnostic Imaging
Most dental insurance plans typically allow:
Bitewing X-rays once per year
Full-mouth X-rays every five years
While these intervals are often sufficient, some patients may benefit from more frequent imaging to ensure accurate diagnosis and optimal care.
Our recommendations are based on:
Your individual oral health needs
Professional standards of care
Your long-term dental health
Insurance coverage may differ from these recommendations. Any services not covered by insurance are the patient’s responsibility.
We encourage patients to become familiar with their specific insurance benefits, as you are personally responsible for all treatment rendered.
For Patients Without Dental Insurance
For patients without insurance:
Payment in full is due at the time of service
We accept cash, check, Visa, and Mastercard
A 5% courtesy discount is available for same day payments made by cash or check
Payment Plans
Payment plans may be considered for patients requiring extensive treatment and are approved on a case-by-case basis.
Please note:
All payment plans must be arranged in advance
Accounts more than 30 days past due are subject to a 15% APR finance charge
